Electronic Signature
I understand and agree that my electronic signature below and its related fields are treated by Nashville Christian School like a physical handwritten signature on a paper form.
• I give permission to Nashville Christian School, Inc. to use whatever emergency measures deemed necessary for the care and protection of my child while under their supervision.
• In case of a medical emergency, I understand my child will be transported to an appropriate medical facility by the local emergency unit for treatment if the local emergency resource deems it necessary.
• I understand that in some medical situations, the staff will need to contact the local emergency resource before the parent, the child’s physician, and/or other adult acting on the parent’s behalf.
• I have been offered/conducted a pre-enrollment tour of the facility.
• I have been provided an electronic copy of the Parent Handbook and can request a paper copy from the administration, and I agree to the terms within.

• I understand my child will receive a curriculum involving age-appropriate personal safety issues as required by the Tennessee Department of Education.
